This position works closely with the RN MDS Coordinator Supervisor in providing assistance and support in the MDS process. Responsible for MDS assessment, formulate resident care plans, participate/lead care conferences as needed, transmit assessments as assigned. Registered Nurse in the State of Illinois in good standing, MDS and Care plan experience required, MDS Certification a plus.
Flexible days 4pm to 7:30pm and weekends. Energetic, self-starter with good communication skills to provide service to our residents. Other responsibilities include set-up, clean up, bussing tables, sweeping and mopping, taking garbage out, and providing outstanding customer service.
Responsible for general cleaning at the Baptist Home to include dining areas, restrooms, and resident rooms. Other responsibilities will be assigned as needed such as taking out garbage, floor cleaning, and special projects. Must have good communication and customer service skills and able to lift a minimum of 75lbs. Hours are Saturday and Sunday, 6am-2:30pm or 2:30pm-11pm as needed.
Responsible to supervise staff on day shift to include, but not limited to, staffing, scheduling, training, performance reviews, and disciplinary action. You will be expected to assist staff as needed by providing hands on support in areas of admissions, med pass, responding to call lights, etc. You will interact with residents, families and physicians responding to inquiries and following up as appropriate. You will be responsible to oversee and implement the Wound Care Program to provide expert knowledge in programming, prevention, treatment, services, and products. You will provide training and coaching as needed and lead department meetings. You will support Person Centered Care as a Change Agent. Registered Nurse in the State of Illinois required, a minimum of 5 years supervisory experience within a LTC environment required, Certified Wound Care Nurse with experience implementing a program required.
Responsible to provide routine nursing care in accordance with established policies, procedures, and nursing standards, and as may be directed by the Unit or Shift Supervisor or the Director of Nursing Services, to ensure that the highest degree of quality resident care is maintained at all times.

The Fairview internship program is a 12-week opportunity that will present you with a variety of training and development experiences. This opportunity provides hands-on experience in critical areas of the manager’s role. We currently have intern opportunities in Human Resources, Marketing, Organizational Development (non-paid), Dining Services, and Life Enrichment. We are looking for senior-level students who are currently enrolled in an accredited undergraduate program at a college or university, preferably working toward a degree. Must have strong organizational and time-management skills. Ability to respond to changing demands is required. Must be proficient with Microsoft Office.
